International Application Checklist

If you are an international participant who will need to apply for a J-1 Visa, please follow these steps to assure that you have prepared all of the necessary applications and documentation:

Complete online Application and Course Selection.
Complete a J-1 Exchange Visitor Information Form (PDF), and email, fax, or mail it to the Summer Institute. We must have your signature on that document.
The following items must also be either faxed or mailed to the Summer Institute:
1. Passport picture page copy for J-1 exchange visitor and J-2 dependents (if any)
2. Financial documentation supporting the amount and source of all non-UM financial support for your attendance at the Summer Insitute
3. If you have held J-1 or J-2 status within past two years:
  - Legible photocopies of all DS-2019/IAP-66, and J visa.
  - If no copies are available, please include a statement specifying the dates you held J status.
4. If you are currently in the US:
  - Copies of your current immigration documents (latest I-94, visa, I-20/DS-2019)
If applying for Kish or Andrews Fellowship, complete Application for a Kish or an Andrews Fellowship and include a copy of your CV or resume
J-1 Exchange visitors must pay the mandatory I-901 SEVIS Fee prior to attending their visa interview. For more details on this mandatory fee please view the US Immigration and Customs Enforcement website.
